Theremin to MIDI

I have a need to use my Ethewave Theremin as a MIDI controller. I want to hot rod the theremin to have CV outputs for the pitch and volume. The next step is converting the CV to MIDI. I have researched this a little and have found a wide variety of price ranges. Does anyone have real world experience with this? Any suggestions?
